Primary Care Services

We are able to provide all of your routine medical care and health maintenance through scheduled visits, as indicated by your personal medical needs. This includes physical exams, prescriptions, labs and other diagnostic tests as needed.

 

Supplemental Primary Care Services

With Supplemental Primary Care Services you retain your own primary care physician and NHome will see you in your home and send note to your physician. This can be helpful with post operative care, if you have a serious medical problem that limits your mobility or you are regaining your strength and conditioning after an illness.

 

Transitional Care Services

This includes post hospital/rehabilitation care for individuals to ensure an appropriate, safe discharge into the community.
